What’s Really in Your Water – And Should It Stay There?

If you’ve ever wondered what’s flowing from your faucet besides H₂O, you’re not alone. From TikTok threads to town hall meetings, one tiny ion is stirring up some serious waves: fluoride.

Once hailed as a public health miracle, fluoride is now under fresh scrutiny. So what’s changed? Is fluoride in drinking water still necessary? Or is it time to rethink this decades-old practice?

Let’s break it down. 

What Is Fluoride and Why Is It in Water?

Fluoride is a naturally occurring mineral that helps prevent tooth decay. Since the 1940s, cities across the U.S. have added it to drinking water as a public health measure—cutting cavity rates by 20-40%, especially in kids.

The science was clear, the benefits were huge, and for a long time, fluoride was considered untouchable in the world of water quality.

Until now.

The Shift: Why Some Communities Are Pulling the Plug

Fast forward to today, and the conversation is changing. Cities like those in Utah are removing fluoride from drinking water. US Health Secretary Robert F. Kennedy Jr. said this week that he plans to direct the CDC to stop recommending fluoridation in communities nationwide.   Why? Here are the main reasons behind the movement:

 1. Concerns About Neurodevelopment

Some recent studies (including one in JAMA Pediatrics) have linked prenatal fluoride exposure to lower IQ scores in children. While the science is debated, it’s raising flags for parents and health-conscious communities.

 2. Dental Fluorosis Rising

Excess fluoride exposure during early childhood can lead to dental fluorosis, which causes white spots or staining on teeth. While mostly cosmetic, it’s fueling skepticism about “how much is too much?”

 3. Personal Choice and Informed Consent

Opponents argue that water fluoridation is essentially medicating the population without consent—a red flag for those who prioritize bodily autonomy.

 4. Cost and Infrastructure

Fluoridation costs an average of $20,000 annually for small-to-medium utilities. Some communities feel that money could be better spent elsewhere—especially when fluoride is already in toothpaste.

The Case for Keeping Fluoride in the Water

So, why do most public health experts still support it?

 1. Proven Protection, Especially for Vulnerable Groups

Fluoridated water reduces cavities significantly—particularly in children, low-income families, and rural communities with limited access to dental care.

 2. It Pays Off

The CDC says every $1 spent on fluoridation saves $32 in dental costs. That’s not just smart science—it’s smart economics.

 3. It’s Endorsed by the Experts

Fluoride is backed by the CDC, ADA, WHO, AMA, and nearly every reputable medical organization.

 4. It’s Equitable

Unlike toothpaste or dental visits, fluoridated water reaches everyone, regardless of age, income, or health literacy.

 What Are the Legal Limits?

The EPA’s Maximum Contaminant Level (MCL) for fluoride is 4.0 mg/L to prevent skeletal fluorosis. There’s also a Secondary MCL of 2.0 mg/L to avoid moderate dental fluorosis in kids under 9.

Most communities fluoridate at 0.7 mg/L, the level recommended by the U.S. Public Health Service for optimal tooth decay prevention.

How Is It Tested?

At SPL we use EPA Method 300.0 with Ion Chromatography to test fluoride levels in drinking water. In 2024, we analyzed over 1500 drinking water samples for fluoride and found 1 sample that had an MCL exceedance of greater than 2.0 mg/L (at 2.39 mg/L).  No samples exceeded 4.0 mg/L.  Our average result was 0.3. mg/L, well below the Secondary MCL.  Accurate testing ensures public water systems stay compliant—and transparent.

But Can’t I Just Use Toothpaste?

Good question. Yes—fluoride toothpaste and mouthwash contain much higher concentrations (1,000–1,500 ppm), but they’re only effective if:

  • You use them properly.
  • You have access to them.
  • You actually brush consistently.

Fluoridated water provides low-dose, around-the-clock protection—no effort required. That’s why many experts say it’s essential alongside fluoride toothpaste, not instead of it.

So… Should Fluoride Stay or Go?

That’s the million-dollar (or 20-thousand-dollar) question.

Supporters say fluoride is a low-cost, high-impact tool to protect public health. Critics argue it’s outdated, unnecessary, or even unsafe. The truth? It’s up to local communities to decide—fluoridation is voluntary, not federally mandated.

But whether you’re for it or against it, understanding the facts beyond the fear is key.
